Your Path LLC, also known as YourPath, is a Minnesota outpatient provider of substance use and mental health care that also offers services in Kentucky. They follow a recovery-oriented, harm-reduction model built around the belief that recovery looks different for everyone, so clients help decide what their own path should look like. Their services include on-demand comprehensive assessments (formerly Rule 25 assessments), ASAM Level 1 outpatient treatment, medications for opioid use disorder, and individual and group mental health therapy. Much of this care is also available through telehealth.

Their Level 1 outpatient program involves fewer than nine hours of services a week, including individual and group counseling, relapse prevention, and recovery planning. It suits people with mild to moderate symptoms, those stepping down from more intensive care, and clients referred by a court or probation. For medication-assisted treatment, they prescribe buprenorphine (such as Suboxone) and naltrexone and can help clients get access to methadone. Peer recovery specialists with lived experience offer support by text, and care coordinators help with referrals to other treatment, medical care, or even employment. In Kentucky, they run outpatient therapy and groups and complete court-referred SB90 diagnostic assessments using tools such as the GAIN-SS, PHQ-9, and LOCUS, and they accept all types of insurance.
