Western Arkansas Counseling and Guidance Center, known today as The Guidance Center, is a nonprofit community mental health center that has served western Arkansas since 1971. In Booneville they operate an outpatient clinic and the Horizon Renewal Center, a residential program for adults with substance use disorders and co-occurring mental health conditions. Their care spans 30- to 45-day residential stays, outpatient services, and a structured Community Bridge step-down for people transitioning back to daily life. Through their wider network, they also offer 24/7 crisis care and a short-term crisis stabilization unit with medical detox in Fort Smith, and they accept Medicaid, Medicare, and most private insurance, with a sliding fee scale so that no one is turned away for inability to pay.

At Horizon, each client works from an individualized plan and meets one-on-one with both a licensed therapist and an addiction counselor, alongside group counseling, cognitive behavioral therapy, motivational interviewing, and trauma-informed care. A signature feature is ground-based equine group therapy, done without riding, where clients practice patience, emotional regulation, and clear communication with horses in real time. When clinically appropriate, medication-assisted treatment is paired with counseling, and APRNs handle psychiatric evaluation and medication management, while mindfulness practice, life skills, and staff-supported community outings round out the daily schedule. Many staff members are in long-term recovery themselves, and aftercare planning starts well before discharge, while the Booneville outpatient clinic provides individual, couples, family, and group therapy in person or by telehealth.
