About Transition House Inc

The Transition House in St. Cloud, Florida, is a nonprofit recovery organization that has spent more than 30 years helping men overcome substance use while rebuilding stable lives. Founded by people in recovery, they use a therapeutic community model built on personal accountability, peer support, and a structured daily environment, with plans tailored to each person’s needs and goals. Their services address addiction alongside mental health concerns such as depression, anxiety, and PTSD. Care includes a men’s residential program, intensive outpatient treatment, and connected services for housing, employment, and reintegration.

The core residential program runs 90 days and includes 20 hours of group therapy each week, an assigned counselor, access to psychiatric evaluation, medical monitoring, and meals. For homeless veterans or those at risk of homelessness, a Grant Per Diem program combines residential substance use treatment with medication management, behavioral health care, vocational training, job placement, and case management aimed at securing permanent housing. Their reentry program serves people leaving prison, pairing housing and employment with substance use treatment, group therapy, education, and skill building in problem-solving, conflict resolution, and relapse prevention. Clients can move between these programs and continue with outpatient care through the affiliated Inspire Counseling and Support Center.
