San Francisco AIDS Foundation is a community health organization in San Francisco, California, whose substance use treatment centers on The Stonewall Project, a state-certified outpatient drug and alcohol program. The Stonewall Project primarily serves gay, bisexual, and queer men, other men who have sex with men, and trans and non-binary people. Their approach is rooted in harm reduction, so clients can pursue full abstinence, abstinence from specific substances, or safer use, with no requirement to stop or change use before getting help. Counseling and group sessions are available both in person and virtually.

In the enrolled counseling program, clients attend at least two peer support groups each week, with individual counseling added when appropriate. For anyone not ready to commit, drop-in groups require no intake and have no wait. Their Positive Reinforcement Opportunity Project (PROP) is a 12-week contingency management program that pairs counseling and community with financial incentives for people looking to cut back on or quit meth, cocaine, or other stimulants. People with opioid use disorder can start Suboxone through a public health street medicine team, get connected to methadone, and join a 30-day incentivized program that helps with intake and staying on medication, with secure lockers available for storing prescriptions. They also offer drop-in harm reduction counseling, hepatitis C treatment with no sobriety requirement, and Enhanced Care Management for San Francisco Health Plan members.
