St. Mark’s Place Institute for Mental Health, also known as UNITAS and now operating as St. Mark’s Institute for Health, is a nonprofit outpatient clinic in Manhattan’s East Village. Dr. Roman Pabis founded it in 1983, and they combine psychiatry, substance use treatment, and primary care under one roof. Their name comes from the Latin word for oneness, reflecting a belief that mind and body should be treated together, and they work with each adult patient toward their own goals. They treat alcohol, opioid, stimulant, and prescription drug problems, including co-occurring mental health conditions, through medically managed outpatient detox, individual counseling, group therapy, family counseling, and aftercare support.

Medication is a central part of their addiction care. They offer buprenorphine (Suboxone) and monthly Sublocade injections for as long as a patient needs them, followed by a gradual taper, along with oral and injectable naltrexone (Vivitrol) for opioid and alcohol use disorders. During outpatient detox from opioids, benzodiazepines, or alcohol, patients recover at home with medication and can reach a doctor around the clock, while therapists draw on approaches such as CBT, DBT, psychodynamic, interpersonal, and couples therapy alongside psychiatric medication management. They also run a New York State DMV-approved program for impaired-driving cases, treat medical conditions like hypertension, diabetes, HIV, and hepatitis C on site, and serve patients in several languages, including Polish, Russian, Ukrainian, and Spanish.
