About Polara Health

Polara Health is a community health organization based in Prescott Valley, Arizona, that has served Yavapai County since 1966 with integrated primary care, mental health, and substance use services. Their care is client-centered and whole-person, addressing physical, mental, relational, and spiritual health while helping each person set recovery goals that feel meaningful to them. They treat dependence on heroin, fentanyl, and other opioids, as well as alcohol and other drugs, often alongside anxiety, depression, and serious mental illness. Levels of care include residential treatment at their Center for Recovery, intensive and enhanced outpatient programs, and Suboxone medication-assisted treatment, backed by a 24/7 crisis stabilization unit and an adult behavioral hospital.

Residential clients take part in individual counseling, group therapy, activity groups, and therapeutic community living, with links to local recovery support groups and educational sessions for families. The IOP runs 36 three-hour sessions held three times a week, drawing on cognitive behavioral therapy, motivational enhancement, twelve-step facilitation, and psychoeducation, and each participant also works one-on-one with a case manager or therapist on legal, financial, and relationship challenges. The enhanced outpatient program combines education and therapy groups three days or evenings a week with required weekly self-help meetings, while court-involved adolescents can attend the Journey Program, an eight-week residential option based on the Seven Challenges model that lets teens keep up with school. Peer support, vocational rehabilitation, psychiatry, and an alumni group for residential graduates help people stay connected and rebuild stability after treatment.
