About Phoenix Domus LLC

Phoenix Domus LLC is a men’s halfway house in the Fairview Hills section of Altoona, Pennsylvania, providing DDAP-licensed residential drug and alcohol treatment at the ASAM 3.1 level of care. They serve up to 16 adult men who have finished inpatient treatment and need a structured, substance-free place to keep building their recovery. Their holistic model looks at the biological, psychological, social, mental, and vocational factors behind addiction, with the broader aim of helping each man return to community life healthy in body, mind, and work.

Every resident completes at least five hours of clinical work each week through individual counseling and group therapy, and attends at least four community recovery meetings, such as AA, NA, MARA, or SMART Recovery. Treatment plans are built around each person’s needs, and residents help shape their own goals. A care coordination team works with outside medical providers, specialists, and community partners, assists with appointments and medication management, and advocates for residents while teaching them to manage the healthcare system on their own. The program also targets educational and vocational gaps so men can return to steady employment, and it prepares them for stable, recovery-supportive housing after discharge.
