Milestone Recovery is a Portland, Maine nonprofit that has served people facing addiction and homelessness since 1967, with services ranging from street outreach and a low-barrier emergency shelter to detox and residential treatment. Their approach centers on meeting people wherever they are and helping them move toward stability, with an emphasis on dignity and on care offered free of charge to anyone who needs it. For substance use, they provide medically supervised detox for adults withdrawing from alcohol or opioids, a 30-day residential program, and an extended care residential program for men lasting up to nine months. Staff with lived recovery experience help each person plan a realistic next step, whether that’s further treatment, sober living, or outpatient care.

Detox is voluntary and includes nursing and clinical assessments, medications to ease withdrawal when appropriate, optional groups three times a day, and regular meals. People can call around the clock to request a bed, though they must complete the screening call themselves, and admissions happen during daytime hours. The 30-day residential program combines individual counseling, several therapy groups each day, life skills work, attention to physical health, access to community self-help meetings, and aftercare planning before discharge. The extended care program in Old Orchard Beach follows a phased model in which men earn independence over time, beginning with short solo outings and moving to weekend visits and passes, with reentry and housing planning starting around the six-month mark.
