About HealthFinders Collaborative

HealthFinders Collaborative is a nonprofit community health center in Northfield, Minnesota, serving underserved and uninsured families across Rice and Steele Counties since 2005. Their licensed 245G substance use program sits alongside primary care, dental, and mental health services, so people can address addiction, co-occurring mental health concerns, and physical health with one coordinated team. Care is trauma-informed, culturally responsive, and offered in English, Spanish, and Somali, with a harm reduction approach that works with people at whatever stage of readiness they’re in. They accept Medical Assistance, MinnesotaCare, and other public plans, offer a sliding fee scale for those without insurance, and turn no one away for inability to pay.

Treatment begins with a comprehensive assessment and can include individual, group, and family counseling, treatment coordination, and medications for opioid use disorder regardless of insurance type. Their Mobile Opioid Support Team offers Rice County residents dealing with opioid or stimulant use one-on-one support, resource referrals, and limited emergency funds, while a free syringe services program provides sterile supplies, naloxone, fentanyl and xylazine test strips, and STI testing. They also partner with Rice County Community Corrections on jail and charge diversion for people facing substance-related charges, and they help with practical barriers like transportation, housing, and insurance navigation. For younger people, they provide prevention, screening, and assessment in Northfield and Faribault schools, run peer recovery support groups for youth ages 10 to 20, and allow teens 16 and older to consent to their own treatment.
