About Harlem East Life Plan

Harlem East Life Plan, often called HELP, is an outpatient health center in East Harlem, New York, that brings addiction treatment, mental health counseling, and primary medical care together under one roof for adults. Their team of addiction medicine doctors, psychiatric providers, counselors, and social workers builds strength-based treatment plans that start from where each person is, using evidence-based practices. Care includes ambulatory medical detox, medication-assisted treatment for opioid use disorder, and individual and group counseling for substance use. They are CARF-accredited and licensed by New York State’s health, mental health, and addiction services agencies, and they offer same-day walk-in services on weekdays.

For opioid addiction, board-certified addiction medicine physicians prescribe methadone, Suboxone, or Vivitrol alongside counseling, and a methadone-to-abstinence track is available for people who want to taper off over time. Outpatient counseling in English and Spanish addresses goals such as sobriety, harm reduction, parenting skills, anger management, domestic violence prevention, and DWI-related treatment, with added support around vocational and educational development. They coordinate with agencies such as Children’s Services, New York City Probation, State Community Supervision, and the DMV, which can help people with legal or family-court involvement. Enrollment specialists also help clients understand and sign up for health coverage.
