About Greater Family Health
Greater Family Health is a nonprofit, federally qualified community health center with a clinic in Hanover Park, Illinois, part of a network of 12 locations across Chicago’s northwest and western suburbs. They began in 1998 as an effort to serve Elgin’s low-income families and now see patients regardless of their ability to pay. Their model brings medical, dental, and behavioral health care together under one care team, so substance use disorder services sit alongside mental health counseling, psychiatric care, and primary care. Their behavioral health care is offered at the Level 1 outpatient level, which suits people who want support while living at home and keeping up with daily life.
Behavioral health services include counseling for individuals, families, and groups, along with psychiatric care, all coordinated with each patient’s other health needs through their Joint Commission–accredited Primary Care Medical Home. When someone needs more intensive help than outpatient care, they refer to regional partners such as Ecker Center, Rosecrance, and Family Service Association, and they connect younger patients to Omni for specialized mental health and substance use services. They also work to remove practical obstacles by offering transportation assistance, bilingual staff and translation, walk-in, same-day, evening, and Saturday appointments, and discounted medications through the federal 340B program at area Walgreens. Most insurance plans, Medicare, Medicaid, and self-pay arrangements are accepted.
