About Fair Haven Community Healthcare
Fair Haven Community Health Care is a community health center in New Haven, Connecticut, that has served local families since 1971 and now operates 21 sites, including 12 school-based health centers. Substance use care is woven into their broader services, which include primary care, mental health, dental, geriatric, midwifery, and HIV/AIDS care. Their addiction medicine team offers medication-assisted treatment, counseling, and support services in a nonjudgmental setting. They serve patients in many languages, including Spanish, and help people with or without insurance find affordable ways to cover their care.
For teens ages 12 to 18 struggling with mental health or substance use, their Adolescent Intensive Therapy Program combines after-school group therapy with weekly individual and family sessions. Each young person starts with a thorough evaluation that shapes a tailored care plan, and staff help them set concrete goals around school performance, peer relationships, and healthy coping skills. Adult patients benefit from having medical care, an in-house pharmacy with delivery and medication packaging, on-site lab testing, and HIV testing and treatment all within one system. When someone needs a higher level of care than they provide, they make referrals to specialists in the Greater New Haven area.
