About Fair Haven Community Healthcare
Fair Haven Community Health Care is a community health center in New Haven, Connecticut, that has served local families since 1971 and now operates 21 locations, including 12 school-based health centers. They fold addiction care into a broader mix of primary care, mental health, dental, pharmacy, and HIV services, so substance use treatment happens alongside the rest of a patient’s health needs. Their addiction medicine team offers medication-assisted treatment, counseling, and support services in a nonjudgmental, outpatient setting. Care is multilingual, including Spanish, and they help both insured and uninsured patients find affordable ways to cover services.
For teens ages 12 to 18, they run an Adolescent Intensive Therapy Program for young people dealing with mental health or substance use challenges. Each teen starts with a full evaluation that shapes a personalized care plan, then attends after-school group therapy along with weekly individual and family sessions. Treatment goals focus on practical progress, such as better school performance, healthier peer relationships, and stronger coping skills. When a patient’s needs go beyond what they provide, they refer families to specialists in the Greater New Haven area, and an on-site pharmacy and laboratory make it easier to coordinate medications and testing.
