About Connected Health

Connected Health is a physician-led outpatient practice in Okemos, Michigan, that combines addiction medicine with gynecologic care under one roof. They treat adults 18 and older of any gender who are living with substance use disorders, including opioid use, alcohol use, and tobacco use, and they approach recovery as part of a person’s overall medical care. The practice was founded by Drs. Lori Burke and Jennifer Hoffman, two OB-GYNs who became board certified in addiction medicine after seeing how poorly care was coordinated for patients with substance use disorders. Their stated aim is to meet patients without judgment and build a plan that fits into everyday life.

For opioid use disorder, they review a patient’s use patterns, past treatment, and health history before discussing medications that ease withdrawal and reduce cravings. People who want to cut back on drinking or quit smoking can explore both medication and non-medication options, and on-site lab work, urine drug screens, STI testing, and vaccines help keep care in one place. Pregnant patients can receive substance use disorder management in coordination with their own prenatal provider, although the practice does not offer prenatal care or deliveries. They also work closely with outside providers and facilities and will connect patients elsewhere when a need falls outside their scope, and they accept most commercial and Michigan state insurance plans.
