Community Partners Integrated Healthcare (CPIH) in Sierra Vista, Arizona, is part of a network of licensed integrated care clinics serving Phoenix, Tucson, and rural communities across central and southern Arizona. They bring primary medical care and behavioral health together under one roof, so someone dealing with substance use, mental illness, or both can work with a doctor, therapist, and addiction counselor on the same coordinated team. Each member chooses a primary care physician and builds an individualized plan with support from psychiatrists, nurse practitioners, recovery coaches, peer support specialists, and case managers. They accept AHCCCS, Medicare, and commercial insurance at several locations, and they also offer interpreter and veterans services.

Outpatient care includes individual counseling, mental health and substance use recovery groups, peer support, tobacco cessation, nutrition education, chronic disease management, and telemedicine. Across the wider organization, Assertive Community Treatment and Forensic ACT teams follow a SAMHSA-based model to serve adults with serious mental illness and co-occurring substance use, including people involved with the justice system.
