About Colorado Coalition for the Homeless

Colorado Coalition for the Homeless is a Denver-based nonprofit that has spent more than 40 years serving people who are experiencing or at risk of homelessness across Colorado, combining housing, health care, and supportive services, including substance use recovery. They believe safe housing and quality health care are human rights, and they ground their work in trauma-informed care that recognizes how often homelessness follows abuse, violence, brain injury, or military service. For people struggling with drugs or alcohol, they offer long-term recovery-oriented transitional housing for up to two years, as well as a low-barrier Medication Assisted Treatment (MAT) clinic in Denver.

Their best-known program, the Fort Lyon Supportive Residential Community, has operated since 2013 on a large campus in Bent County in southeastern Colorado, where about 250 residents work toward recovery together. Residents build their own plan from peer support led by people with lived recovery experience, education, vocational training, employment services, and a range of hobbies and activities, and they receive help finding housing and work when they return to their chosen community, sometimes with housing subsidies. The same model is used at Sage Ridge, a newer campus in the Denver metro area that accepts people from across the state, and entry to both campuses requires a referral from a case manager, homeless service provider, or health care provider. At the Stout Street Health Center, the walk-in MAT clinic pairs medical treatment with behavioral health support, one-on-one peer support, and peer groups for ongoing recovery.
