Chase Brexton Health Care is a nonprofit community health center in Baltimore, Maryland, offering outpatient substance use treatment alongside primary care, dental, mental health, and specialty services across five Maryland locations. Their approach looks at the whole person, including physical health, mental health, and life circumstances, and they welcome people who are still using; no one has to be substance-free before starting. Treatment centers on medication for alcohol and opioid use disorders, peer recovery support, and therapy for co-occurring mental health concerns. Patients can choose whichever pathway fits them, and finishing one option is never a requirement for starting another. They accept most insurance plans, Medicare, and Medicaid, and they work with people who are uninsured.
Their STEP Clinic pairs a medical provider with a Certified Peer Recovery Specialist. Visits happen every one to four weeks and include a brief physical exam, a medication check-in, and a prescription that can be filled at the on-site pharmacy. Walk-ins are accepted during set hours, though STEP participants need to be, or become, Chase Brexton primary care patients. Through the PRISM program, peer specialists with their own lived recovery experience meet with patients weekly to talk through goals, relationships, and struggles. Care with a behavioral health therapist begins with a one-on-one assessment to build a personal plan, and a social work team helps primary care patients with housing, money, and food needs that can affect recovery.
