About Centennial Mental Health Center Inc
Centennial Mental Health Center is a nonprofit community behavioral health provider serving the rural counties of northeastern Colorado, with an outpatient office in Elizabeth among ten locations across the region. For more than 40 years, they have offered mental health and substance use disorder care, guided by the belief that reaching out for help takes strength. Their substance use services are open to anyone age 11 and older and include outpatient counseling, medication-assisted treatment with Suboxone for opioid dependency, and sober living residences. Care isn’t limited to the office, either, since staff also work with clients in private homes, nursing facilities, supported housing, correctional institutions, and other community settings.
Outpatient programming covers Dialectical Behavior Therapy skills, relapse prevention, Seeking Safety groups for people working through trauma alongside substance use, and Strategies for Self-Improvement and Change. They also offer specialized treatment for women, Level II education and therapy, the Special Connections program, and urinalysis monitoring. For people who need a stable, substance-free place to live, Mark’s Place and Carly’s House in Fort Morgan provide separate residences for men and for women, and women may bring dependent children. These voluntary stays usually last three to six months and include peer support, on-site addiction treatment, and case management, while residents pay modest fees, share household chores, and hold jobs, look for work, or pursue education or training.
