Centennial Mental Health Center is a nonprofit community behavioral health provider serving rural northeastern Colorado, with Burlington among their ten outpatient offices. For more than 40 years, they have offered mental health and substance use disorder care, believing that reaching out for help shows strength rather than weakness. Their substance use services are open to anyone 11 and older, and care can take place in their offices, private homes, nursing facilities, supported housing, correctional settings, and other community locations. Alongside addiction treatment, they provide adult mental health, crisis, integrated health, prevention, and youth and family services.

Outpatient substance use programming includes Dialectical Behavior Therapy skills, Level II education and therapy, relapse prevention, Seeking Safety groups, Strategies for Self-Improvement and Change, Special Connections, and specialized treatment for women. They also offer urinalysis monitoring and medication-assisted treatment with Suboxone for opioid dependency. For people who need a stable place to live while building recovery, they run two voluntary sober living residences in Fort Morgan, one for men and one for women that can accommodate dependent children. Residents typically stay three to six months, receive on-site addiction treatment, case management, and peer support, pay modest program fees, share household chores, and work, look for work, or attend school or training.
