B & D Integrated Health Services is a nonprofit community health center in Durham, North Carolina, with a second office in Wake Forest, that brings primary medical care and behavioral health treatment together under one roof. Founded in 2005 by Lauren Durant, PhD, a licensed clinical psychologist and addictionologist trained at Duke University Medical Center, they care for children, teens, and adults facing mental illness, co-occurring substance use disorders, and chronic health conditions. For addiction, they offer a substance abuse intensive outpatient program, supported by outpatient therapy, psychiatric medication management, and community-based teams. Their approach centers on flexible, family-focused, and culturally responsive care, and patients sit on their Board of Directors to help guide the organization.

Therapy is available in individual and group formats, and adults in recovery can be paired with certified peer support specialists who draw on their own lived experience with mental health or substance use. Tailored care management assigns a single care manager to build a plan that covers medical and mental health needs as well as practical barriers like housing, food, and transportation, and eligible Medicaid members can get help arranging non-emergency rides to appointments. Younger clients can access intensive in-home services, family therapy, and pediatric primary care that screens early for behavioral health concerns. They accept Medicaid, Medicare, and private insurance, welcome uninsured and underinsured clients, offer sliding-scale fees of $25 to $160 per visit, and provide most behavioral health services at no cost to households at or below 300% of the Federal Poverty Level.
