Adapt Integrated Health Care is a community health organization based in Roseburg, Oregon, that has served southern Oregon for more than 50 years with primary care, mental health care, prevention, and addiction treatment under one roof. Their addiction services include an 11-bed sub-acute residential detox program for adults withdrawing from alcohol, opioids, or other drugs, adult residential treatment at The Crossroads, and an opioid treatment program offering methadone and buprenorphine. They treat substance use alongside co-occurring mental health conditions, and no one is turned away for inability to pay, with a sliding fee scale based on family size and income.
The Crossroads, which has offered residential care for adult men and women since 1982, builds an individualized plan for each resident and provides onsite medical care with round-the-clock nursing, along with programs for chronic pain management and tobacco dependence on a smoke-free campus. Residents also have access to family programming for children and loved ones, transportation to medical, dental, and child visits, and cultural and recreational activities. In detox, physicians and nurses handle assessment, medication management, and medical monitoring, while counselors provide individual and group sessions, with intake coordinated through the Rapid Access team. The CARF-accredited opioid treatment program, with clinics in Roseburg and North Bend, pairs medication with psychiatric care, case management, relapse prevention education, and trauma-informed counseling, and each level of care connects patients to ongoing support as they move through recovery.
